To order by mail please print your information on the
order form & mail with your check.
Go up to the top left of your browser then go to file then go to print.
If you do not have anything printing from your printer, you must change the print settings in your browser.

In your browser go to file then go into print setup, make sure black text & also black lines is selected with a check mark in the box beside these attributes. Cut this form between the doted lines and mail with check.
